For 2025-2026 school year, the Michigan City High School Athletic Department will continue to partner with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms and the required IHSAA Physical Information form for your student-athlete(s) that is required for athletic participation. FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. More importantly, this web-based program will allow our athletic trainers and coaches access to emergency contact information and medical consent forms in an emergency event at the touch of their smart phones. We view this process as safer for our student athletes and their families. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update(s). To get started, click on the Parent Playbook (Link below) to get started in FinalForms.
