A completed IHSAA physical form signed after 4/1/25 must be uploaded into the student-athlete’s FinalForms account prior to participating in any workouts / practices beginning after July 1st, 2025. If you have not yet registered with FinalForms, you can do so here: https://michigancity-in.finalforms.com/ Student-athletes who are missing information will not be allowed to participate in workouts/practices once summer workouts resume following the IHSAA Moratorium week. Do not wait to submit physical forms just before a scheduled workout/practice as they must be able to be double checked and processed at least 24 hrs ahead of time. The IHSAA allows pre-participation exams to be signed after April 1st for the subsequent academic year. Ex) April 1, 2025 satisfies IHSAA exam requirement through June 2026.
